Catchment area
Before filling out a registration form, check you are in our catchment area.
You must live in a postcode area of PA14 or PA15 2.
Find out which GPs you can register with on the NHS inform website.
To register with the Practice:
- Download and complete an online registration form from the NHS inform website. Send it back to us via email on [email protected] or post it to us
- Visit reception when we are open and collect a paper registration form
When you register, it’s helpful to have your NHS or CHI number.
When you register, an appointment will be arranged for you to see the practice nurse so that they can do a health screen, which assists the doctor in his/her first consultation with you.
At that time of registration, please also provide details of any medication that you take, when either emailing your registration form or when you hand in/post it.
